The consultation form collects your full name, age, phone, message and processing consent. The concern field may contain health information. Include only details needed for your request; no medical documents are required.
Purpose and access
Details are stored in this site’s WordPress database to respond and coordinate a consultation. Only site administrators can access requests. The theme does not email request details or send them to external services.
Retention and deletion
Call +995 591 97 94 94 to withdraw a request or ask for deletion. The site operator determines the necessary retention period and deletes records through WordPress administration. Hosting backups may have separate retention periods.
Technical information
A temporary IP-hash record is stored for up to 10 minutes for abuse prevention. The theme does not automatically load advertising or analytics trackers. WordPress and hosting may maintain technical logs.
Your choice
